Information Request
WCD has developed an Information Request Form for use when requesting information.
The Information Request Form allows WCD to:
- Understand and respond to the request
- Follow Iowa law governing confidential information and public records; and
- Provide responsive information.
Submit a completed Information Request Form to expedite the request process.
Access the online portal to request information from WCD.
Authorization to Release Information Form
Some records relating to workers' compensation claims are confidential under Iowa law. An individual may allow release of confidential information relating to a workers' comp claim by completing the form.
WCD cannot release confidential information to you without a completed and signed authorization to release information form.
This form does not authorize any entity other than WCD to release information.
After contacting WCD using the online portal, submit a completed Authorization to Release Information form.
Release of Information
WCD releases information only upon receipt of payment for all fees related to the request.
WCD will provide all requested records in PDF or mp3 format unless delivery by mail is requested.
The information is released once WCD receives payment. Requesters can access their information through their online portal account.
Expedited Release of Information
WCD processes information requests based on their order and complexity. Requesters can expedite their requests ahead of others by paying an expedited release fee. WCD prioritizes expedited release requests over regular ones and fulfills them in the order received.
Expedited releases are exclusively available through the online portal for quicker processing. WCD aims to fulfill these requests within three business days. Check the State of Iowa holidays if unsure about timing.
However, WCD might reject an expedited release if gathering the requested information takes too long.
Payment
WCD will not release requested information until full payment for the request is made. Requesters can make payments online or by check.
