How Do I Apply for a Psychologist Health Service Provider Certification?

Documentation and questions may be submitted by email: plpublic@idph.iowa.gov.

Applicants must be a licensed Iowa psychologist to be eligible to receive a Health Service Provider (HSP) certification. 

1. Verification of current registration at the doctoral level by the National Register of Health Service Providers in Psychology, OR  
2. Proof of two years of supervised clinical experience.  See Iowa Administrative Code  645— 240.7 for specific requirements.                            
3. Applicants must request the following forms be completed and submitted to the Board office for approval. 
   1. Internship:  Organized Health Services Training Program Confirmation Form (629.75 KB) .pdf  
   2. Postdoctoral: Supervisor Confirmation Form   (187.58 KB) .pdf

Additional documentation may be requested by the Board office upon receipt of the completed forms.
