How Do I Apply for Manufactured Home Manufacturer Licenses?

1. Complete the Application for Manufactured Housing Manufacturer License.
2. Mail the completed application to the department with:
   • sample certificates of origin for each separate make;
   • check for license fee.

Note: incomplete application packets will not be processed and will be returned to you by mail.

Mail COMPLETED application packets to the department.
