Application for a Provisional Psychology License

The application for a provisional psychology license must be submitted on paper. There are two options to complete the application.

  • Fillable Form: Download the form to complete on your computer or other device.  You will need to print the application to sign it in ink prior to submitting for processing. You will need to open this document in an application that is not a browser to complete the application electronically.
  • Standard PDF Application: Print the PDF and complete it in writing.

Residency Requirements

Predoctoral residency 

  • Copy of the applicant’s acceptance letter for the predoctoral internship.  

Postdoctoral residency 

  • Official transcripts conferring a doctoral degree in psychology from an institution accredited by the North Central Association of Colleges and Secondary Schools or an equivalent accrediting association or entity in other regions of the U.S., sent directly to the Board office from the college or university.    

The psychology program from which the doctoral degree was received must be one of the following: 

  • Accredited by the American Psychological Association;  
  • Accredited by the Canadian Psychological Association;  
  • Designated by the Association of State and Provincial Psychology Boards (ASPPB)/ National Register Designation Project as a doctoral program in psychology; OR  
  • The applicant must hold a specialty diploma by examination from the American Board of Professional Psychology.  

Foreign-trained applicants should contact staff for educational requirements.  

Supervision Plan Requirements

A completed Supervision Registration Form must be received before approval to take the EPPP exam will be granted.
