A license or registration is required before you begin any regulated activity.
- Applications are valid for six months, except for:
- Pharmacist license by license transfer – valid for one year
- Pharmacist license by examination or score transfer – valid for two years
- Application fees are nonrefundable and nontransferable
- A low-income fee waiver is available with supporting documentation
- A military service fee waiver covers the initial fee and one renewal, with documentation
Each applicant must report any criminal history or disciplinary action.

The Board will email a renewal notice 60 days before your license or registration expires.
If you do not renew on time:
- A late fee equal to the application fee will apply during the 30-day grace period
- After 30 days, a reactivation fee, usually four times the application fee, will apply
