How Do I Apply for a Salon License?

Create an account, apply, and pay online using the online portal.

Documentation may be submitted electronically to the board office as follows:

The application fee is $84. The fee may be paid online. All application fees are nonrefundable.

A salon license is not transferable. A change in ownership of a salon will cancel the existing license and require a new application. Note: A salon cannot be sold if disciplinary actions are pending.

Required Notifications to Board

Licensees are required to notify the board of changes in any of the following:

• Changes in name or address.  The licensee shall return the original certificate and make a request, including payment of the fee, for an updated certificate of license.
• Upon sale of a salon, the owner must return the license certificate and report the sale of the salon to the board within 10 days.
  • The board may request legal proof of the transfer of ownership.

In compliance with IAC 645--Ch. 63 salons must meet requirements for infection control in salons.

Note: The salon may be inspected for compliance with sanitation rules within 12 months following the issuance of the salon license.

Universal Precautions

Requirements include, but is not limited to, the following:

• Have biohazard sharps containers for disposing of any sharp needles, blades or instruments.
• Have disposable gloves available.
• Disinfectant for instruments and implements of the profession.
• Used instruments and supplies must be placed in a proper receptacle clearly labeled "used".  All used items must be kept separate from disinfected and ready-to-use items.
• Have bloodborne exposure procedures.
• Have infection control methods for creams, cosmetics and applicators.
• Have proper laundering and clean storage available.
