Definitions Related to the Practice of Dentistry
Practice of dentistry as defined in Iowa Code section 153, and other definitions that apply to dental practice.
“Practice of dentistry” includes the rendering of professional services in this state as an employee or independent contractor or the rendering of any dental decisions, including diagnosing, treatment planning, determining the appropriateness of proposed dental care, or engaging in acts that constitute the practice of dentistry. The following classes of persons shall also be deemed to be engaged in the practice of dentistry:
- Persons publicly professing to be dentists, dental surgeons, or skilled in the science of dentistry, or publicly professing to assume the duties incident to the practice of dentistry.
- Persons who perform examinations, diagnosis, treatment, and attempted correction by any medicine, appliance, surgery, or other appropriate method of any disease, condition, disorder, lesion, injury, deformity, or defect of the oral cavity and maxillofacial area, including teeth, gums, jaws, and associated structures and tissue, which methods by education, background, experience, and expertise are common to the practice of dentistry.
- Persons who offer to perform, perform, or assist with any phase of any operation incident to tooth whitening, including the instruction or application of tooth whitening materials or procedures at any geographic location. For purposes of this paragraph, “tooth whitening” means any process to whiten or lighten the appearance of human teeth by the application of chemicals, whether or not in conjunction with a light source.
“Conscious sedation” means moderate sedation.
“Deep sedation/general anesthesia” is a controlled state of unconsciousness, produced by a pharmacologic agent, accompanied by a partial or complete loss of protective reflexes, including inability to independently maintain an airway and respond purposefully to physical stimulation or verbal command.
“Minimal sedation” means a minimally depressed level of consciousness produced by a pharmacological method that retains the patient’s ability to independently and continuously maintain an airway and respond normally to tactile stimulation and verbal command. Although cognitive function and coordination may be modestly impaired, ventilatory and cardiovascular functions are unaffected. A patient whose only response reflex is withdrawal from repeated painful stimuli is not considered to be in a state of minimal sedation.
“Moderate sedation” means a drug-induced depression of consciousness, either by enteral or parenteral means, during which patients respond purposefully to verbal commands, either alone or accompanied by light tactile stimulation. No interventions are required to maintain a patent airway and spontaneous ventilation is adequate. Cardiovascular function is usually maintained. Prior to January 1, 2010, moderate sedation was referred to as conscious sedation.
“Nitrous oxide inhalation analgesia” refers to the administration by inhalation of a combination of nitrous oxide and oxygen producing an altered level of consciousness that retains the patient’s ability to independently and continuously maintain an airway and respond appropriately to physical stimulation or verbal command.
Dentists licensed in this state shall not administer moderate sedation, deep sedation, or general anesthesia in the practice of dentistry unless they have obtained the appropriate sedation permit from the board.
Dentists licensed in this state shall not administer nitrous oxide inhalation analgesia in the practice of dentistry until they have complied with the provisions of the IAC 650--Chapter 29. Note: A sedation permit is not required for the administration of minimal sedation (anxiolysis).
Licensure and Examination Requirements
Overview of licensing & examination requirements in Iowa for a dental or dental hygiene license.
A permanent dental or dental hygiene license authorizes the holder to practice in the state of Iowa. Requirements for obtaining an Iowa license are found in Iowa Administrative Code 650--Chapter 11.
Note: Students enrolled in a CODA-accredited dental or dental hygiene school are exempt from the requirements for licensure for all activities directly related to the curriculum and activities of the accredited program.
Dentists and dental hygienists enrolled in a residency program in Iowa must hold an active license, or apply for and be issued a resident license before practicing clinically.
Graduates of a CODA-accredited dental or dental hygiene school and who have successfully completed a board-approved clinical examination within the previous five (5) years may apply for a dental or dental hygiene license in Iowa on the basis of examination.
Note: Applicants who have been licensed for one (1) year or longer in another state, district or territory must apply by credentials or verification.
Graduates of a CODA-accredited dental or dental hygiene school and who have a current, valid license in another state that was issued on the basis of clinical examination OR has at least three (3) years of current, active practice in that state may be eligible may apply for license by credentials.
Note: Applicants who have been licensed for one (1) year or longer in another state, district or territory must apply by credentials or verification.
A dentist or dental hygienist who holds an active license in another state may be eligible for licensure on the basis of verification. Refer to Iowa Administrative Code 650--Chapter 11 for specific requirements.
Applicants for a permanent dental or dental hygiene license issued on the basis of examination must have successfully completed one of the following clinical examinations:
Central Regional Dental Testing Service, Inc. (CRDTS)
Western Regional Examining Board, Inc. (WREB)
States Resources for Testing & Assessments (SRTA)
Commission on Dental Competency Assessments (CDCA)
Council of Interstate Testing Agencies, Inc. (CITA)
American Board of Dental Examiners (ADEX) as administered by CDCA, WREB, SRTA or CITA.
*Beginning in May 2021, the Iowa Dental Board accepts the patient-based and manikin-based clinical examinations.
Information about taking a clinical exam, including the exam schedule and testing procedures, should be obtained directly from the testing agencies.
Remediation Requirements
Please note that applicants who fail the clinical licensing examination two or more times must submit proof of additional formal remedial education that is approved in advance by the Board prior to each subsequent exam attempt. See Iowa Administrative Code 650--Chapter 12 for more information.
Graduates of Foreign Dental Schools
To be eligible, graduates of foreign dental schools must complete a postgraduate general practice residency of at least one (1) year, or two (2) years of undergraduate supplemental education at an accredited dental school. While the foreign dental school graduate does not need to have a D.D.S. or D.M.D. degree, the applicant must submit evidence showing that the educational requirements have been met. A diploma, degree or certificate must be awarded to the applicant upon successful completion of the program.
About half of the accredited U.S. dental schools consider graduates of foreign dental schools for advanced standing admission – meaning the gradate may be eligible to be admitted to the regular dental degree program as a second or third year student. A graduate of a foreign dental school who does not meet the requirements specified above may be eligible to apply for a waiver of these rules. In general, the Board has approved waiver requests in cases where the applicant has completed at least one year of a general practice residency program at an accredited dental school. Effective Jan. 1, 2021, pursuant to HF 2627, individuals whose household adjusted gross income does not exceed 200% of the federal poverty level may request a waiver of application fees related to an initial application for license or registration. Please note: The fee waiver does not extend to applications for renewal, reinstatement or reactivation.
To determine eligibility, you must first identify the number of individuals in your household. Once this is determined, reference the Federal Poverty Level and identify the income that matches the number of individuals in your household. If your household adjusted gross income, as reported on your most recent federal tax return, is less than the identified value, you are eligible for the one-time fee waiver.
DOCUMENTATION FOR QUALIFICATION OF WAIVER
If you wish to apply for a fee waiver you must attest that your household adjusted gross income does not exceed 200% and agree to submit documentation of one of the following to verify your household income:
1. If you are married and filed separately, you will need to submit the Federal Tax Return for both your spouse and yourself.
2. If you were claimed as a dependent on another person’s tax return, you will need to submit the Federal Tax Return for the tax filer who claimed you as a dependent.

Reinstatement applications require payment of a reinstatement application fee and one past-due renewal fee. Additionally, dentists and dental hygienists must submit a background check packet for which the fee is $46.

VOLUNTEER
If you will be volunteering dental services at a non-profit and you do not hold an active Iowa dental or dental hygiene license, you may apply for a temporary permit on the basis to volunteer. There is no fee for this application; though a completed application must be submitted via the Iowa Dental Board online services website.
EDUCATIONAL or URGENT NEED
Temporary permits on the basis of educational purpose or urgent need may be issued for up to three months at a predetermined location of practice. Applicants are limited to a maximum of three temporary permits in total. Thereafter, applicants would be required to apply for a permanent dental or dental hygiene license.
Note: Temporary permits on the basis of educational purpose or urgent need are not intended for individuals who will make application for a permanent dental license.
