Attach Verification of Registration
Email the documents to plpublic@idph.iowa.gov if there are any issues attaching.
- Copy of current registration card issued by the Commission on Dietetic Registration (CDR), OR
- Copy of web-based verification from the Commission on Dietetic Registration website indicating the applicant holds registration status, OR
- Letter sent to the Board office by the Commission on Dietetic Registration verifying the applicant holds registration status.
Applicants licensed in another state
Attach or email verification of license held in other state.
The license verification must include:
- License issue date
- Expiration date
- Any pending or past disciplinary action
