The Iowa Board of Pharmacy (β€œBoard”) took the following actions at its most recent meeting held May 3, 2022.

Heather Gleason, Technician, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  Technician Gleason agreed to voluntarily surrender her technician trainee registration to resolve the allegation that she diverted prescription drugs from a pharmacy for personal use.

Heather Oberg, Technician, entered into a Settlement Agreement and Final Order.  Technician Oberg was issued a citation and warning to resolve the allegations contained in the Notice of Hearing and Statement of Charges.

Cheveux, LLC, Phoenix AZ, entered into a Settlement Agreement and Final Order. The Board issued a citation and warning, imposed a civil penalty of $2,500, and requires the pharmacy to undergo an on-site inspection by one or more Board representatives within the next twelve (12) months to resolve the allegations contained in the factual circumstances of the Settlement Agreement and Final Order.

Optum Infusion Services 501, Inc, Las Vegas, NV, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with operating a pharmacy without a current pharmacy license.  The Board issued a citation and warning and imposed a civil penalty of $1000.

Walgreens 07455, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with dispensing, or contributing to the dispensing of, an incorrect prescription and for violating Board rule 657β€”8.3(3)β€œa” which requires a pharmacy to ensure that the pharmacy employs an adequate number of qualified personnel commensurate with the size and scope of services provided by the pharmacy.  The Board issued a citation and warning, imposed a civil penalty of $750, and requires the pharmacy to establish a staffing plan.
