The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held March 1, 2022.

Lindsey Collett, RPh, Pharmacist applicant. The Board approved Ms. Collett’s pharmacist license by Consent Agreement.  The Board issued a citation and warning to Ms. Collett and requires her participation in the Iowa Monitoring Program for Pharmacy Professionals (IMP3) for prior disciplinary action against her Kentucky pharmacist license related to division of controlled substances.

Katelynn Parmer, Technician Trainee, entered into a Settlement Agreement and Final Order.  Technician trainee Parmer agreed to voluntarily surrender her technician trainee registration to resolve the allegations contained in the Notice of Hearing and Statement of Charges.

Arnold Beukelman, DVM, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged Dr. Beukelman with failing to maintain a CSA registration and for failing to create and maintain complete and accurate records.  The Board issued a citation and warning, required the completion of continuing education pertaining to the recordkeeping requirements for CSA registrants, and imposed a civil penalty of $2,000.

Lori Lindstrom, MD, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged Dr. Lindstrom with violating the Board rule which outlines the requirements for maintaining the unique authentication credentials issued to the prescriber for utilization of the electronic prescription application and for violating the Board rule which requires each user registered to access PMP information to securely maintain the login and password and any other secure access credentials assigned to the individual user.  The Board issued a citation and warning, required Dr. Lindstrom to submit written policies and procedures governing electronic prescribing and PMP access within thirty (30) days, complete necessary education and training on the use of electronic prescribing platforms and the PMP, and imposed a civil penalty of $2,000.  Dr. Lindstrom also agreed to audit all transmitted prescriptions for a period of two years to ensure each prescription was authorized.

Walgreens 05060, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with violating Board rules pertaining to a change of pharmacist in charge and the Board rule which requires an inventory of all controlled substances following the close of business the last day under the terminating pharmacist in charge’s employment.  The Board issued a citation and warning and imposed a civil penalty of $1,500.

Walgreens 05721, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with employing or continuing to employ a person to assist a pharmacist with technician functions associated with the practice of pharmacy who is not currently registered as a pharmacy technician.  The Board issued a citation and warning and imposed a civil penalty of $750.

Walgreens 05852, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with employing or continuing to employ a person to assist a pharmacist with technician functions associated with the practice of pharmacy who is not currently registered as a pharmacy technician and for violating the Board rule which requires patient counseling for a new prescription.  The Board issued a citation and warning and imposed a civil penalty of $1,500.

Cheveux, LLC, Phoenix AZ, the Board issued a Notice of Hearing regarding the pharmacy’s appeal of the Notice of Intent to Deny Renewal.  A hearing is scheduled for May 3, 2022.
