The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held January 11, 2022.
Demetria Simpson, Pharmacy Support Person (PSP) applicant. The Board approved Ms. Simpson’s PSP registration by Consent Agreement. The Board requires Ms. Simpson to notify any current pharmacy employers and any future prospective pharmacy employers (at the time of employment interview) of the Agreement.
LeAnne Hicks, Technician Trainee applicant. The Board approved Ms. Hick’s technician trainee registration by Consent Agreement. The Board requires Ms. Hicks to notify any current pharmacy employers and any future prospective pharmacy employers (at the time of employment interview) of the Agreement.
Erica Evans, DPM, CSA applicant. The Board approved Dr. Evans’ CSA registration by Consent Agreement. The Board issued a citation and warning to Dr. Evans for a 2018 disciplinary action against her Nebraska podiatrist license for fraudulently prescribing narcotics and a diagnosed opioid use disorder and subsequent Consent Agreement issued by the Iowa Board of Podiatry in 2019.
Randy Nicoski, RPh, Pharmacist applicant. The Board approved Mr. Nicoski’s pharmacist license by Consent Agreement. The Board issued a citation and warning to Mr. Nicoski for 2019 disciplinary action against his Oregon pharmacist license for failing to ensure the pharmacy was in compliance with the laws and rules governing the practice of pharmacy.
Precision Direct Rx, Omaha NE, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Board charged the pharmacy with operating a pharmacy without a current pharmacy license. The Board issued a citation and warning and imposed a civil penalty of $5,000.
Five Star Compounding Pharmacy, Clive IA, entered into a Combined Statement of Charges, Settlement Agreement and Final Order. The Board charged the pharmacy with failing to properly submit information to the PMP and for dispensing an incorrect prescription. The Order sets forth parameters related to the sale of the pharmacy and requires the surrender of license 1185 upon closure of the pharmacy.
Kindra Kruse, RPh, voluntarily surrendered her pharmacist license due to allegations of failure to comply with the terms of her Iowa Monitoring Program for Pharmacy Professionals (IMP3) contract.
Cheveux LLC, Phoenix AZ, the Board voted to deny the licensee’s renewal application for failing to comply with rules of the Board.
November 10, 2021 Board Orders
Katelynn Parmer, Technician Trainee. The Board issued a Notice of Hearing and Statement of Charges against technician trainee Parmer. A hearing is scheduled for March 1, 2022.
Nortru LLC, Wholesale Distributor applicant, Detroit MI. The Board approved the wholesale distributor’s license and CSA registration by Consent Agreement. The Board issued a citation and warning and imposed a civil penalty of $1000 for operating as a reverse distributor for prescription drugs, including controlled substances, without a wholesale distributor license or CSA registration.
