The Iowa Board of Pharmacy (β€œBoard”) took the following actions at its most recent meeting held May 1, 2019:

Pharm D Solutions, Nonresident Pharmacy.  Pharm D Solutions entered into a combined Statement of Charges, Settlement Agreement, and Final Order with the Board.  The Order resolved charges of distributing non patient-specific prescriptions without an outsourcing facility license and compounding essentially copies of approved drugs by agreeing to not seek renewal or reactivation of its nonresident pharmacy license.

Dan Pomeroy, Pharmacist.  Pharmacist Pomeroy entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board.  The Order resolved charges of failure to secure the pharmacy in the absence of a pharmacist, allowing prohibited activities in the absence of a pharmacist, and having an unregistered pharmacy support person.  The charges were resolved by issuing a citation and warning and requiring re-taking and passing the MPJE.

Coon Rapids Pharmacy, Pharmacy.  Coon Rapids Pharmacy entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board.  The Order resolved charges of failure to secure the pharmacy in the absence of a pharmacist, allowing prohibited activities in the absence of a pharmacist, and having an unregistered pharmacy support person.  The charges were resolved by placing the pharmacy on probation for a period of one year with terms and imposing a civil penalty in the amount of $2,500.00.

Loretto Hucker, Technician.  Loretto Hucker entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board.  The Order resolved charges of working without a current registration.  The charges were resolved by issuing a citation and warning and imposing a civil penalty in the amount of $100.00.

Walgreens #02445, Nonresident Pharmacy.  Walgreens #02445 entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board.  The Order resolved charges of contributing to a dispensing error.  The charges were resolved by issuing a citation and warning, imposing a civil penalty in the amount of $5,000.00, and requiring the pharmacy to follow its policy regarding Iowa prescriptions for compounded preparations.

David Schlichtemeier, Pharmacist.  Pharmacist Schlichtemeier entered into a Settlement Agreement and Final Order with the Board.  The Order resolved charges of unprofessional behavior.  The charges were resolved by issuing a citation and warning, imposing a civil penalty in the amount of $1,000.00, and requiring two hours of continuing education pertaining to ethics.
