At the August 29-30, 2017 meeting of the Iowa Board of Pharmacy (Board), the following public actions were taken:

Stoner Drug. The Board filed a Notice of Hearing and Statement of Charges against the pharmacy’s license along with an Order to Show Cause against the pharmacy’s CSA registration, charging the pharmacy with failure to perform an annual inventory, failure to maintain receipt and disbursement records, improper disposal of controlled substances, and failure to maintain drug supplier invoices. An administrative hearing is scheduled for October 31, 2017.

Phillip Kuhr. The Board filed a Notice of Hearing and Statement of Charges against the pharmacist for violating the duties of a pharmacist in charge. An administrative hearing is scheduled for October 31, 2017.

Charles Robinson. The Board entered into a Settlement Agreement and Final Order issuing a Citation and Warning, assessing a $2,000 civil penalty, and requiring the completion of an eighteen (18) hour Patient Safety and Medication Error Prevention continuing education course within 60 days.

Martha Morin. The Board entered into a Settlement Agreement and Final Order that includes five (5) year probation for Ms. Morin’s pharmacist license with conditions.

Thorson Drug. The Board entered into a Settlement Agreement and Final Order in which the pharmacy was placed on probation for five (5) years with conditions.

Walgreens Pharmacy. The Board entered into a Settlement Agreement and Final Order in which the pharmacy’s license was placed on probation for three (3) years with conditions.

Peterson Drug. The Board entered into a Settlement Agreement and Final Order which included a Citation and Warning and $5,000 civil penalty.

Mark Vogt. The Board entered into a Settlement Agreement and Final Order which included a Citation and Warning and $10,000 civil penalty.

LaDona Hart. The Board entered into a Settlement Agreement and Final Order with this technician trainee in which she voluntarily surrendered her registration.

Jeffrey Killian. The Board entered into a Combined Statement of Charges, Settlement Agreement, and Final Order against Mr. Killian’s pharmacist license following public action against his pharmacist license in California. Mr. Killian’s Iowa license will be on probation for the duration of his probationary period in California, with conditions.
